Residents receive their ambulatory care training at Open Door Family Medical Centers, a federally qualified health center founded in 1972, committed to delivering quality medical, dental, behavioral and specialty health care as well as chronic disease management, to low-income people in Westchester County, NY.  Open Door is accredited by the Joint Commission and licensed by the New York State Department of Health as a diagnostic and treatment center.  At the core of Open Door’s philosophy is that every patient is a member of a care team of highly trained, certified family physicians and other clinical staff. In 2017, Open Door served 53,876 patients, including 14,663 children and 693 newborns, for a total of 258,622 patient visits.”   Every year Open Door serves well over 50,000 patients including a large number of pregnant women and children.   

Open Door is a patient-centered medical home, accredited by the National Committee for Quality Assurance (NCQA), where patients are at the center of their care team and communication is based on trust, respect and shared decision-making. In a medical home, electronic medical record systems make it possible to track patient test results, review medicines and follow up with providers involved in the care of the patient. Patients and their families participate in the decision-making process with their medical team.

Open Door is also a participant of Taconic Health Information Network and Community (THINC).  THINC is a non-profit organization that brings together the community and articulates a collaborative vision for improving the quality, safety and efficiency of health care in the Hudson Valley.  Open Door shares in THINC’s vision that health information technology is the foundation that leads to quality improvement, patient safety and improved outcomes, which ultimately benefits the entire community.

Convenient Locations

Open Door has  community health centers located in the Westchester communities of Ossining, Port Chester, Mamaroneck, Mount Kisco, Brewster and Sleepy Hollow. In addition, Open Door operates seven school-based health centers in Port Chester and Ossining.

Medical Care

In addition to primary care, chronic disease management services are provided for illnesses such as diabetes, hypertension, asthma and HIV.  The care that Open Door providers is coordinated and centered on the patient’s specific needs. Open Door providers are all board certified or board eligible in their specialties.

Patients often need help understanding the information given to them by their provider at their medical visits.  To assist patients, each Open Door site has dedicated patient advocates who are available to provide help with insurance coverage and referrals, advice about chronic illnesses, education on a group or individual basis, and assistance with other documents or questions.  These services are provided in a culturally and linguistically appropriate manner.  The electronic medical record (EMR) used at Open Door eClinicalWorks or eCW, allows for better communication among all team members.  The EMR also enhances the performance improvement initiatives undertaken at Open Door, and allows the providers to benchmark their clinical outcomes with partner organizations as well as compare these outcomes against national standards.

Dental Care

Open Door provides dental services for adults and children.  Dental services include full dental examinations, oral cancer screenings, sealants for children, dental cleanings, fillings, dentures, tooth removal, emergency care and rapid HIV testing for our patients.  Prevention is an important part of care; therefore, Open Door dentists and hygienists provide patient education on caring for teeth and gums for all patients with a specific focus on our diabetic patients, pregnant mothers, and babies’ new teeth. In 2018, these services expanded to our Sleepy Hollow location.

Behavioral Health Services

Behavioral health services at Open Door include psychiatry, individual therapy, family therapy and play therapy for children as well as preventative screenings. Extensive communication exists between a patient’s primary care physician and the behavioral health department, facilitated by the electronic record which makes this behavioral health information available to all providers and primary caregivers working with the family.


Specialty services provided at Open Door include optometry, podiatry, psychiatry, dermatology, HIV care, women’s health and ultrasound.
